Q2 2026 reporting
About 12 minutes. Seven short sections. Your organization's name is used only for duplicate detection and is never published. No login required.
Before you start — common questions
Is this anonymous? Can the state see who submitted?+
Yes, anonymous in the public record. Your organization name is collected for de-duplication only and is never published. Only authenticated coalition reviewers can see raw submissions, under a written confidentiality agreement. The Department of Health and Welfare, the Legislature, MCOs, and the public cannot access individual responses.
What if I'm the only provider of my type in my county?+
We apply cell suppression — no figure is published from fewer than five providers, and no category or regional breakdown is published from fewer than three. Single-provider cells roll up to broader groupings or are withheld for that quarter. Your data still strengthens the statewide totals.
Is there any legal or contractual risk in submitting?+
We do not collect Protected Health Information, billing claims data, or anything tied to a specific participant. Submissions are operational metrics about your organization. Providers should still check their own DSP, MCO, or accreditation contracts; the coalition cannot give legal advice. The privacy policy is written to be forwarded to a board or compliance officer.
Do I need an account?+
No. The survey is open without login. An account is only required for coalition reviewers.
How precise do my numbers need to be?+
Best available estimate is fine. Aggregate medians and percentages are robust to ±10% noise on individual responses. If you genuinely don't track a value, choose 'Not tracked' rather than guessing.
What if my answers change after I submit?+
Email privacy@hcbsimpact.org with your organization name and submission date to amend or withdraw. Aggregates are recomputed within 48 hours.
Can I save and come back later?+
Yes. Your answers auto-save to this browser as you fill them in. Come back to this page on the same device and click Continue — your draft is restored automatically. The draft is cleared after a successful submission.
